End of Life Directions for Aged Care (ELDAC)

The ELDAC project provides information, guidance, and resources to health professionals and aged care workers to support palliative care and advance care planning to improve the care of older Australians.

Advance care planning conversations can often be difficult for staff to initiate. The participation of three of our Anglicare services, including two residential homes and a home care service, in the ELDAC Aged Care project has helped to build the confidence and capacity of our staff as they care for residents at the end of their life journey.

Among other initiatives, aged care staff at Anglicare Southern Queensland have developed a palliative care and advance care planning working group to support our ongoing improvement in this area.

One of the clinical nurses involved in the project, from St Martins at Taigum, commented that: “Since the commencement of the ELDAC Linkages program we have improved our connection with the specialist palliative care team and we have a better understanding of their role.”
