End-of-Life Pathway

The End-of-Life Pathway, a program under Support at Home, provides additional support for people who have been diagnosed with 3 months or less to live and wish to spend their final weeks at home. It provides funding for nursing and other care services to help maintain comfort, dignity and quality of life.

About End-of-Life Pathway funding

If eligible for this funding, you will receive $25,000 over a 12 week period to cover increased needs for aged care services at home. If you have remaining funding available after the 12 week period, you can continue using it for up to 16 weeks total (an additional 4 weeks).

Eligibility and application process

To be eligible for this program, you must be:

  • Aged 65 years or older (50+ for Aboriginal and Torres Strait Islander people)
  • Diagnosed by a medical practitioner or nurse practitioner as having 3 months or less to live
  • Have a score of 40 or less on the Australian-modified Karnofsky Performance Status (AKPS) scale.

Step 1

Complete an End-of-Life Pathway Form

Ask your doctor or nurse practitioner to fill in the End-of-Life Pathway Form, available on the My Aged Care website.

Step 2

If you are already receiving Support at Home services, you can choose either of these options:

Option A
  1. Give the completed form to your current aged care provider.
  2. Ask them to request an urgent Support Plan Review for the End-of-Life Pathway.
  3. They will upload the form for you
Option B
  1. Call My Aged Care on 1800 200 422, or visit an Aged Care Specialist Officer at Services Australia service centre.
  2. Ask for access to the End-of-Life Pathway, and provide them with the completed form.

If you are not currently receiving Support at Home services, you can choose any of the following options:

Option A
  1. Ask your doctor to upload the completed form through My Aged Care. This will start a high-priority aged care assessment.
Option B
  1. Apply online through the My Aged Care website for a high-priority aged care assessment.
  2. Provide the assessor with your completed form.
Option C
  1. Call My Aged Care on 1800 200 422 or visit an Aged Care Specialist Officer at Services Australia.
  2. Ask for access to the End-of-Life Pathway and provide the completed form.
Step 3

If you are approved:

You will receive a Notice of Decision and a support plan. You can begin commencing services.
