Our nurses provide professional wound care in your home, helping to manage and treat a variety of wounds, including:

- Skin tears, cuts, and abrasions
- Stoma care and management.
For more complex wounds, our wound clinics are available via GP or nurse referral.
Dementia care

Who is eligible for Support at Home (Home Care Package) funding in Qld?
You may be eligible if you’re 65 or older (or 50+ for Aboriginal or Torres Strait Islander people) and need support to live independently. Eligibility is determined through a My Aged Care assessment.
How much do you get through Support at Home (Home Care Package) funding?
Participants receiving services under Support at Home will contribute based on an assessment of their income and assets. The amount varies depending on the type of service:
- Clinical Supports: No contribution
- Independence Services: Moderate contribution
- Everyday Living Services: High contribution
Who is eligible for home assist in QLD?
Home Assist is open to Queensland residents aged over 60 years old or people with a disability who have a place to stay and hold a concession or pension card.
What is the difference between My Aged Care and Support at Home (Home Care Package)?
My Aged Care is the entry point — it manages assessments and referrals. Support at Home (Home Care Package) is the funding you may receive after an assessment to pay for in-home services and nursing support.
How long does it take to get Support at Home (Home Care Package) funding?
Typically, it takes 3 to 6 months to receive Support at Home funding after application. Wait time varies depending on your demand and your assessed priority.
