About AT-HM funding
Funding for the AT-HM scheme is available in three tiers:
- up to $500 for minor items
- up to $2,000 for moderate modifications
- up to $15,000 for major modifications
Eligibility and application process
To be eligible for this program, you must be:
- Aged 65 years or older (50+ for Aboriginal and Torres Strait Islander people).
- Complete an assessment through My Aged Care and be approved for an assistive technology funding tier and/or a home modifications funding tier.

Call 1800 200 422 or visit myagedcare.gov.au to register and book an assessment.

Get assessed
The assessment team will evaluate your needs and determine what funding you are eligible for.
Step 3
Choose a service provider
Once you receive you My Aged Care Letter confirming your funding, you can select a provider.
