About Restorative Care Pathway funding
The Restorative Care Pathway is a time-limited, goal-oriented program that provides tailored rehabilitation services through a multidisciplinary team, including physiotherapists, occupational therapists, and nurses. It aims to help you recover from illness, surgery, or a decline in function, allowing you to stay independent at home. If eligible for this program, you can receive care for periods up to sixteen weeks, across two non-consecutive episodes a year.
Additional services can be added to the program for a fee.
Eligibility and application process
To qualify for the Restorative Care Pathway, you must be:
- Aged 65 years or older (50+ for Aboriginal and Torres Strait Islander people)
- Experiencing a decline in function and are at risk of needing long-term care
- Complete an assessment through My Aged Care and receive approval for the Restorative Care Pathway.
To apply:
Step 1
Contact My Aged Care: Call 1800 200 422 or visit myagedcare.gov.au to check your eligibility
Step 2
Complete a My Aged Care assessment. After registration, a My Aged Care assessor will visit your home or complete an assessment over the phone to determine the type of support you need.
Step 3
If you are eligible, you can discuss your service provider preferences and arrange care.
