We support you and your family through an outreach service in the home. This creates a safe and positive environment for you to discuss your experiences, needs and objectives. We will work with you to help you solve problems and achieve your goals.

  • Parents and caregivers with children and young people up to the age of 18
  • Live in the catchment area.

Are there any costs or out of pocket fees?

No, there are no costs to participate. Assessment Services Connect is funded primarily through the Department of Children, Youth Justice and Multicultural Affairs.

Do you provide services in the home?

SFS provides an outreach service in the home so that families feel safe to discuss their experiences within their own environment. This work includes spending time in the family home to discuss the families goals and how we can support the family to achieve these goals.

How exactly will this program benefit my family?

We will work with all members of the family to understand your story, strengths and concerns. We will then help you identify your goals and the steps that will be required to achieve them.
