About Support at Home funding
Support at Home funding is allocated based on an individual’s care needs and allows recipients to choose an approved provider, like Anglicare Southern Queensland, to manage and deliver services. Support at Home offers long term, comprehensive support, and the funding includes care management, personal care, nursing, allied health, and domestic assistance to help you maintain your independence.
Additional fees may be required based on financial assessment and individual circumstances.

Eligibility
To qualify for Support at Home funding, you must:
- Be aged 65+ (or 50+ for Aboriginal and Torres Strait Islander people)
- Require ongoing, coordinated care to live safely at home
- Complete an assessment through My Aged Care and receive approval.

Check your eligibility

Step 2
Contact my aged care
Call 1800 200 422 or visit myagedcare.gov.au to register online.
Step 3
Arrange an assessment through My Aged Care.
This assessment will determine your eligibility and the level of care you need.
Already been assessed with My Aged Care? Call us on 1300 610 610 and we can support you to access available services that suit your personal needs and goals.
Step 4
Complete an assessment through My Aged Care.
Your assessment will take place in your home, or over phone or video call.
They’ll assess you on factors that determine your eligibility for Support at Home funding, such as your health, how you’re managing at home and, if you’re currently receiving support.
You will receive an outcome letter within 2-6 weeks of your assessment, outlining which Support at Home level you have been approved for and approximate wait time for the funding to be assigned.
Step 5
Receive your funding allocation
You will be assigned a funding level based on your care needs.

Choose an approved provider
Contact Anglicare to develop a care plan tailored to your needs. Call us on 1300 610 610 when your funding is approved or assigned.
Step 7
Receive ongoing care
Your funding is flexible and can be adjusted as your needs change.

Is this funding option suitable for me?
Support at Home funding is ideal if you require ongoing support with daily tasks, health management, or personal care. By completing an assessment through My Aged Care, they will determine if you need high level care, such as Support at Home, or entry level support, accessible through the Commonwealth Home Support Programme (CHSP).

How much funding do I receive?
Your funding classification determines how much financial support you receive. After your My Aged Care assessment, you’ll be assigned a funding level from 1 to 8. The higher your funding level, the more funding you will receive, with Level 1 offering the lowest amount and Level 8 offering the highest amount.

*Contributions will be applied on a tapered rate based on the participant’s income and assets assessment.
