Home Care Package (HCP)

A Home Care Package (HCP) is Federal Government funding designed to support older Australians with more complex care needs to stay in their home for longer. It provides a flexible, coordinated approach with four levels of care, ranging from basic to high-level support.

About HCP funding

HCP funding is allocated based on an individual’s care needs and allows recipients to choose an approved provider, like Anglicare Southern Queensland, to manage and deliver services. HCP offers long term, comprehensive support, and the package includes care management, personal care, nursing, allied health, and domestic assistance to help you maintain your independence.

Additional fees may be required based on financial assessment and individual circumstances.

Levels of Support

There are four levels of Home Care Package subsidies:

  1. Level 1: Basic care needs (fortnightly subsidy = $409.92*)
  2. Level 2: Low-level care needs (fortnightly subsidy = $720.86*)
  3. Level 3: Intermediate care needs (fortnightly subsidy = $1,568.98*)
  4. Level 4: High-level care needs (fortnightly subsidy = $2,378.60*)

Eligibility

To qualify for an HCP, you must:

  • Be aged 65+ (or 50+ for Aboriginal and Torres Strait Islander people)
  • Require ongoing, coordinated care to live safely at home
  • Have an Aged Care Assessment Team (ACAT) approval.

Step 1

Check your eligibility

Step 2

Contact My Aged Care to register online or call 1800 200 422.

Step 3

Arrange an ACAT assessment through My Aged Care. An ACAT assessment will determine your eligibility and the level of care you need.

Already been assessed with My Aged Care? Call us on 1300 610 610 and we can support you to access available services that suit your personal needs and goals.

Step 4

Complete an ACAT assessment

Your ACAT assessment will take place in your home, or over phone or video call.
They’ll assess you on factors that determine your eligibility for Home Care Package funding, such as your health, how you’re managing at home and, if you’re currently receiving support.

You will receive an outcome letter within 2-6 weeks of your ACAT assessment, outlining which Home Care Package level you have been approved for and approximate wait time for the funding to be assigned.

Step 5

Receive your package allocation: You will be assigned a package level based on your care needs.

Step 6

Choose an approved provider: Contact Anglicare to develop a care plan tailored to your needs. Call us on 1300 610 610 when your funding is approved or assigned.

Step 7

Receive ongoing care: Your package is flexible and can be adjusted as your needs change.

Is this funding option suitable for me?

HCP is ideal if you require ongoing support with daily tasks, health management, or personal care. If you only need basic assistance, CHSP may be a more suitable option. Or if you only require short-term support, while you rehabilitate from illness or injury, STRC might be appropriate.

Not included:

  • Full-time residential care
  • Services beyond allocated funding limits.
