Are you eligible for a Home Care Package?

A Home Care Package (HCP) is Federal Government funding that helps older Australians receive care and support while continuing to live at home. Once assigned, you can choose an approved provider, such as Anglicare Southern Queensland, to arrange the services you need.

Step 1

Contact my aged care

Call 1800 200 422 or visit myagedcare.gov.au to register and check if you need an in-home assessment. If you’re referred for a Home Care Package, prepare a list of tasks you need help with – this will ensure a smooth and effective assessment process.

Step 2

Get assessed

The Aged Care Assessment Team (ACAT) will evaluate your needs and determine what services you qualify for. Be open about your challenges so you receive the right level of support. Your prepared list from Step 1 will help ensure all your needs are considered.

Step 3

Choose a service provider

Once you receive your My Aged Care Letter confirming your Home Care Package, you can select a provider. Choosing Anglicare means gaining access to a dedicated Concierge team member who will listen to your goals and guide you in selecting the right services.

Step 4

Receive ongoing support

If you choose Anglicare, an experienced team member will meet with you to coordinate and manage your care. We’ll arrange a regular team of carers to support you, ensuring your needs are met with consistency and compassion.
