How to access Help at Home

It’s comforting to know that there is support available for Australians who want to grow old in their own home. My Aged Care is the government entry point to the aged care system and funding where our older Australians have their needs assessed and access service providers like Anglicare Southern Queensland.
1. Getting ready

Start by contacting My Aged Care by calling 1800 200 422 or visiting myagedcare.gov.au to register, and determine if an in-home assessment for a funding package will be required. If you are directed to a funding package, we suggest you prepare a list of things that you need help with so you can maintain living at home. This will ensure you are prepared for the assessment process.

2. Assessing your needs

The Aged Care Assessment Team (ACAT) assess what services might be available to you. It’s important that you’re honest in your responses so you can receive the support you need. Your prepared list of the things you need help with from Step 1 will assist with the process and ensure all your concerns are covered.

3. Finding a service provider

Once you have received your My Aged Care Letter assigning you a home care package, you are ready to select a Service Provider. By choosing Anglicare, you will have a dedicated Concierge team member who will listen to your goals and discuss with you various options for services.

4. Support provided

If you choose Anglicare as your service provider for home care services, we will immediately arrange for a professional and experienced Client Liaison to visit your home and provide you with ongoing care management, support and coordination. This will include coordinating a regular team of carers to visit you at home to support your needs.
