Fundraising Collection Statement

  1. The Corporation of the Synod of the Diocese of Brisbane (“Anglican Church Southern Queensland” or “ACSQ”) is collecting personal information from people who wish to make a donation to Anglicare Southern Queensland.
  2. The primary purpose of collecting this information is to allow ACSQ and Anglicare to administer and record such donations.
  3. ACSQ and Anglicare collects, uses, holds and discloses personal information in accordance with the Privacy Act 1988 and the Australian Privacy Principles (APPs).
  4. If ACSQ does not obtain personal information from you, it may not be possible for you to make a donation in your name. Of course, ACSQ is happy to accept a donation towards the Anglicare Appeal made anonymously.
  5. Personal Information may be temporarily disclosed to others for administrative purposes only, and retained by ACSQ and Anglicare to enable us to contact you in the future.
  6. Additional information collected on this form will be used for fundraising purposes for the ACSQ and Anglicare Southern Queensland.
  7. Access to personal information is dealt with in accordance with Anglicare Southern Queensland’s Privacy Policy. You may seek to access and correct any information collected about you by contacting the General Manager of ACSQ, and you may make a complaint in accordance with Anglicare Southern Queensland’s Privacy Policy if you believe the ACSQ or Anglicare has breached the APPs.
